Initial Assessment
A Care Coordinator from Fairfield Family Care will visit with you and your loved ones to evaluate your condition, and discuss options in order to create a Plan of Care.
Development of a Plan of Care
We create a Plan of Care based on recommendations from our own initial assessment of the family and client, and the input of case managers, providers, and your physicians.
We create this plan so that our clients know all the services required, potential equipment and supplies needed, the proposed frequency, cost of each service, and the possible payment sources.
Benefit Determination
It is our job to try and identify any programs, community resources, or other such benefits that might be available for our clients such as: VA pension, Long Term Care Insurance, Private Health Insurance, and others.
OUR COORDINATION OF CARE PROGRAM IS A FREE SERVICE TO OUR CLIENTS.
