Coordinating In-Home Care

Initial Assessment

A Care Coordinator visits you or your loved ones home to evaluate condition, discuss options, as well as help to develop a Plan of Care.

Benefit Determination

It is our job to try and identify any programs, community resources, or other such benefits that might be available for our clients such as: VA pension, Long Term Care Insurance, Private Health Insurance, and others.

Development and Ongoing Plan of Care

We create a Plan of Care based on recommendations from our own initial assessment, case managers, healthcare providers, and of course your physicians. We develop this plan so that our clients know all the services required, potential equipment and supplies needed, the proposed frequency, cost of each service, and the possible payment sources.

  1. It is possible that this process may start before the client is discharged from their hospital, rehab or other healthcare facility.
  2. The Plan of Care will be determined by us working with the family and client, the attending physician, and the discharged planner to agree on the key goals to best maximize the healing process.
